Mine got somewhat better if I don't preview anything, but I still see it sometimes. And with the number of typos I make it's bad when I don't preview!

I didn't even have to preview (I think? can't swear I didn't preview, but I usually don't), I just quoted my old post with list made with smileys, and it automatically inserted those extra codes.

I may have to turn off smilies for a bit to work on the problem. But frankly, I don't know what the problem may be. I'll try to get back to this now that my own computer is behaving better.

I'm still having the problem. I keep a copy of my message on my computer. If I want to edit here, I delete the whole old text on the board and use the copy on my computer. Therefore it's okay for me. Testing in BBCode and switching between BBCode and Preview: before editing: I wanted to add the code here and typed :: aniwave :: without the spaces. When I changed to Preview it inserted instead [img src="http://i1308.photobucket.com/albums/s605/tossmoder/emoticons/aniwave.gif"] The editor shouldn't do that with code. It's code!! I'll now hit "Create Post" and then "Edit". I saved the text above so I can keep the original.
After hitting edit: " style="max-width:100%;"] [img src="http://i1308.photobucket.com/albums/s605/tossmoder/emoticons/aniwave.gif" style="max-width:100%;"]" style="max-width:100%;"]
I hope this helps. Maybe others have observed some rules of (mis)behaviour, too. Another observation: I remember that formerly the image-tag turned to ::imagename:: when changing from Preview to BBCode which sometimes took a bit of time. Right now I don't see this.

flotsam, I'm just throwing this out there, 'cuz this is what I did & it worked, but have you tried rebooting? sidestep, do you mean restarting the computer, refreshing the page (F5) or emptying the cache? Flotsam's post captured the problem nicely. I have done all those things Flotsam mentioned above, for different reasons, and non of it helped this problem.

I came back about 2.5 hours ago and started it then. Did you see the change before or after? If afterwards, I'll just head to bed and hopefully see the change tomorrow (I'm in Europe). ETA: Just saw sidestep's ETA and OnTheMend's post.

I haven't touched it yet, sidestep. Sorry. I'm working on a project and not getting much else done. I suspect there is a glitch in the plugin we use for our extra smilies and will have to contact the creator to actually fix it.
