It may be too late in the game to say this … but regarding all the clothes: you said that he keeps himself neat and tidy and clean, but you're finding squalorous clothing around the house and laundering stuff to see if it's okay.
I wonder if you should just throw the stuff away. I laundered tons of stuff from my inlaws' house … especially in the basement, I found so much stuff that was in good shape except for being musty. I wasted a lot of time, electricity, and water trying to get that musty smell out … often you just can't.
I had an aunt who lived in a dreadful squalorous hoard. When EMTs had to come in, they reported the conditions, and the hospital refused to discharge her back into those conditions. The family had to clean up for her. Some authority attempted to bring charges against my dad because of how his sister was living, but that didn't actually happen.

Thanks everyone. I appreciate all of your kind words. It's hard, but needs to be done. It seems to be helping. I've even got the dog to behave quite well! Think I may have overdone it a bit today. we have a bulk roadside pickup and I grabbed lots of stuff from the back porch and sleep out. he was a bit funny about it. So I let him retrieve some stuff, and stopped pushing on the couple of tvs that should go. Still a bit to go, but it's getting difficult, as the bulk,easy to get rid of stuff it's pretty much all done. On the up side, I think a cleaner could come in and not freak out So I can concentrate on just visiting, and maybe helping with some of the tricky little jobs. I haven't broached the power thing again, and it seems ok in my very non professional opinion. But we'll need to get it sussed if a cpap machine is needed for the sleep aponea. Thanks to whoever suggested it Having you all in my corner means SO much! Please know I am thinking of everyone here going through all their stuff and sending you all positive vibes too! xxxx

Hello Ench Darl, don't think I sent you or Twix a welcome. I'm an Aussie, too. Well, from reading all these posts, it seems that things are getting better. But to answer the original question 'is giving a hoarder an ultimatum the best solution?' I would have said probably not, because it would force them to do the clean up - it hasn't come from themselves. It's the 'you made me cleanup' scenario, but it appears that your uncle has realised that cleaning up is the best for him. You know, sometimes people get into a squalor situation and for what ever reason, know that they are in a dire situation, but can't get out of it by themselves, and, what's more to the point, don't know how or where to find help.

Hi ench. I just saw this thread now. What a true blessing you are for your uncle. Your kindness has been life-changing (and probably life-saving) for him. Your parents must be so relieved that you stepped in. I hope he allows some sort of an electrical inspection - even if it's just for everyone's peace of mind. And he/ you can arrange for someone to clean on a regular basis.
